Partners In Care is Hiring a Physical Therapist Near Central, OR

Join us and make a difference in the lives of our patients! Partners In Care is seeking a Physical therapist to support our home health and hospice patients. The Physical Therapist plans, coordinates, implements, and evaluates home health and hospice therapy services to meet the needs of patients and their families. 
Compensation: $36.00-$45.47 (DOE). 
Location: Central Oregon
Schedule: Monday-Friday 8:00 am-5:00 pm (full-time). 
Travel: Local/regional travel by personal vehicle during the working day, centralized from the Bend office and extending throughout Central Oregon. Mileage reimbursement is provided.

What You Get to Do: 
  • Complete an initial comprehensive (if HH includes OASIS) and accurate assessment of patient and family to determine home health needs.
  • Develop, monitor, and revise individualized care plans, establishing measurable goals based on the therapy assessment, including the patient’s functional abilities and limitations, rehab potential, and the patient’s diagnosis.
  • Collaborate with the patient/family and physician in the planning process.
  • Initiate appropriate preventative and rehabilitation treatment and procedures.
  • Provide physical therapy instructions and HEP education to patient/family/caregivers as appropriate. Adapt teaching styles to meet patient and family needs.
  • Actively promote and participate in the Fall Intervention Program for patients.
  • Ensure that arrangements for equipment and other necessary items and services are available and provided.
  • Identify discharge-planning needs as part of the care plan development and implement them prior to discharge of the patient.
  • Develop, monitor, and update the Home Health Aide Plan of care as necessary.
  • Instruct, supervise, and evaluate home health aide care provided every 14 days.
  • Communicate and coordinate with all members of the healthcare team to coordinate the care plan, including community resources.
  • Attend staff meetings, case conferences, and IDT as required. 
  • The above statements are meant to be a representative summary of the duties of the physical therapist position. 
Required Skills & Qualifications: 
  • A degree from a baccalaureate or master’s program in physical therapy approved by an accredited organization.
  • Licensed to practice as a physical therapist within the state.
  • Two years of appropriate experience as a physical therapist. Community/home health experience is preferred.
  • Ability to pass a background test, drug screen, and TB test.
  • Valid CPR Certification. 
  • Current automobile insurance, valid Oregon driver’s license, and reliable transportation.
  • Strong communication and interpersonal skills.
  • Ability to work independently and collaboratively within an interdisciplinary team.
  • Compassion, empathy, and dedication to patient care.
